HEP C PRISON
TRANSITION SERVICE
Our Prison Transition Service supports people living with Hepatitis C who are currently in prison, just released from prison or returning to prison.
FACE-TO-FACE SESSIONS
Our prison workers will see you in prison for a confidential talk around Hep C testing/treatment either while being inside or when released. Will talk about risk, prevention and harm reduction.
Plan for individual support and linkage to services once released to minimise stress that may occur when leaving prison.
FOLLOW UP AFTER LEAVING PRISON
If you have left prison and need support continuing or starting you Hep C treatment our Harm Reduction Case Managers are here to help. They can:
- Link you in with a Hep C prescriber if you need treatment/testing after release.
- Offer you individual case management and refer you in with other community services if you need it.
- Help you to get a test done to see if you have been cured from the virus.
REFERRALS
We take referrals from your-self, medical staff, QCS staff, re-integrations service and community services.
If you have started treatment before entering prison, we will take referrals from your treatment provider in the community to make sure you continue your treatment while being inside.
