Privacy Policy

 

QuIHN is committed to protecting your privacy and does so in, as per the requirements of the Privacy Act (Commonwealth) 1998. QuIHN will take reasonable steps to protect your privacy.

QuIHN's Needle and Syringe Program services are completely anonymous and confidential. QuIHN NSP's will collect some de-identified data (such as age, drug(s) used, disposal methods, Indigenous status and postcode) for reporting and statistical purposes.

In some service instances, such as accessing our Medical services, our Treatment services or where you may be receiving case management, QuIHN will need to collect some personal information to assist us to provide you with services. However, QuIHN will treat such personal information with the utmost respect for privacy requirements and will not release this information to any other parties without your express written approval or where required by law to do so.

In some circumstances QuIHN is legally obliged to disclose information about you in the following circumstances:

  • where the Police / Court orders QuIHN to make records or information available (e.g. subpoenas and search warrants);
  • where a client poses danger to themselves or others; and
  • where a child or young person is suspected of being harmed or is at risk of harm as defined by the Child Protection Act.

QuIHN collects private information to assist in providing an appropriate service to you and discharge its duty of care and other legal responsibilities.

QuIHN may collect personal and private information and hold them on both electronic and hardcopy files.

QuIHN may collect information from you directly, or from people that you have given consent to release information to QuIHN.

Your personal information is documented (for example, in case notes and treatment plans) and used to make decisions such as your eligibility for services and your treatment plan.  Personal information is reviewed at case conferences and in supervision.

QuIHN may release de-identified personal and sensitive information for reporting purposes.  Your personal information will not be shared with third parties by our staff or contractors, without your permission; unless QuIHN is legally obligated (see above); or you have a current Authorisation to Gain or Release Information.

Requests to access information can be made in writing.  If you feel information contained in client records are incorrect, you can request for them to be amended.  The outcome of such requests will be provided to you.

QuIHN Website(s)

QuIHN websites collect two types of information. The first type is anonymous information. The web server makes a record of your visit and logs the following information for statistical purposes:

  • the user's server address;
  • the user's top level domain name (e.g. com, .gov, .net, .au, etc.);
  • the date and time of the visit to the site;
  • the pages accessed and documents downloaded;
  • the previous site visited; and
  • the type of browser used.

No attempt will be made to identify users or their browsing activities except, in the unlikely event of an investigation, where a law enforcement agency may exercise a warrant to inspect the internet service provider's logs.

Another way information may be collected is through the use of "cookies". A cookie is a small text file that the website may place on your computer. Cookies may be used, among other things, to track the pages you have visited, to remember your preferences and to store personal information about you. You can adjust your Internet browser to disable cookies or to warn you when cookies are being used. However, if you disable cookies, you may not be able to access certain areas of the website.

Our websites may contain links to other websites and social media pages including Facebook, Twitter and LinkedIn. We are not responsible for the privacy policies of the entities responsible for those websites and we recommend that you review the privacy policies applicable to any other websites you visit.
